My background is ER and ICU. I've worked in major teaching hospitals like the trauma ER at The University of Pennsylvania and The National Naval Medical Center in Bethesda. I've also worked in small rural hospitals and did international relief work.<span style="mso-spacerun: yes;">  </span>I've worked in large for profit facilities as well as local nonprofit and government hospitals. I've also had the good fortune to live and work in Thailand for 5 years, where I met my amazing wife. Why is that: </strong>All of my transports are interesting, each one with unique challenges. The most interesting was probably transporting a woman home who took ill while on a family cruise. She was facing a terminal illness and knew her time was short. She became very talkative during our trip and told me about her life, a very interesting story involving multiple continents, the golden age of Hollywood and how proud she was of her very talented children. She didn’t say but it was my belief she may have told me things she had never told anyone else, even her family. Her family contacted me just a few days after I got her home to tell me she passed away at home with her children at her side. Miami, JFK, Istanbul, Toulouse, Frankfort, Seoul, Brisbane, LAX, Miami. 52 hours in the air over 6 days. Lay overs in airport infirmaries in Germany and Korea. 6 different lift trucks plane/deplane processes. The client had suffered multiple traumas, was on a stretcher, and required constant care. All transitions went without a hitch, the ground personnel where very helpful, the cabin crews on the different carriers were great. His family was very appreciative. “We were so afraid he would never get home again.” The client had a long recovery ahead of him but he was home with family and friends. It has always been my philosophy that compassion is the key ingredient to being a great nurse, therefore, I would say that helping my patients get safely back to their homes after an unpleasant experience takes precedence. The look of relief on their faces when I first walk into a room is so gratifying, but the gratitude that they express when the transport has been completed is priceless. The wonderful places I get to travel to and explore are also very rewarding. I make sure I get to see as much as I can of everywhere I travel.With the present state of our medical system, nurses are completely overwhelmed with tons of paperwork and too many patients. She hadn’t been taking her medications as prescribed and subsequently had a psychotic episode landing her in a psych ward of a hospital near Pisa. When I walked into the ward, it was like a scene from "One Flew Over the Cuckoos Nest". Every person in there appeared to be very over sedated.My patient could not walk, she could not articulate, or form even a simple sentence. In the 5 days she was in the hospital, they hadn’t given her any soap, a toothbrush or toothpaste, no shampoo, no change of clothes or even a comb. I had to get her cleaned up before we flew. The trip was very complicated and it had been a horrible experience for her. Since she had seen almost nothing on her trip, I asked the driver to take us by the Leaning Tower of Pisa where we got her out of the car and took a nice picture with her standing in front. When we got to her brother’s house, she and her family were so relieved that she was home safely. They were all in tears and I got lots of hugs! I was lucky in the fact that I had a few hours and got to see a few of the historic areas of Rome. My client and his companion were a great help in giving me directions to get around on the train system. On the flight back, I was able to assist another passenger on Air Italia. The flight crew had paged for any medical personnel for assistance. They knew I was there but they didn't want to bother me. I let them know that I was willing to help. As I watched "3 Doctors" take care of this passenger, I went and offered my assistance, as it was obvious to me that they were having some problems. They were most grateful that I came and gave them some help. The passenger did fine in the end. However, it turns out that the 3 Doctors were dentists en route to a convention in Ft. Lauderdale!! The next day, with clear skies, we flew in an 8-seat, piston engine airplane over snow-covered mountains. This flight was an unforgettable experience with its beautiful views. Upon arriving in the small village, I was greeted with warmth and hospitality by the patient's family. This made the trip forever memorable. That transport ironically happened on July 4, 2009. He was robbed and suffered a gunshot wound to the head, causing paralysis on one side of his body. Although our communication attempts were poor, I quickly understood that he was suffering from severe muscular spasms from being bedridden for so long. I had nothing to give him---no one ever predicted this. I eventually took him to the clinic in the Mexico City airport to be seen by a physician where they were able to medicate him to get him to Veracruz, his hometown. My patient was in her 40's with a history of COPD from years of tobacco abuse and she was hospitalized with Congestive Heart Failure and pneumonia. The hospital that the patient was currently at was below standards and very poor quality. It was dirty, sheets were soiled for days, bathrooms without soap or toilet paper, trash cans overflowing.....not what I would have expected. I assessed my patient, she was begging to leave. I found that she was fit to fly and escorted her and her husband to the car waiting outside for us. Everything went smoothly until we reached 30,000 feet or so. She went into flash pulmonary edema and she didn't respond to any of my treatments. I had to fight to get the stewardess to understand how serious my patient's situation was until a physician on board agreed with me that she needed to get to a hospital stat. We landed in Miami (on superbowl night, mind you) and got her to the nearest hospital. Below is Isabel’s perspective of flight nursing:</p> <p><strong>What is your most enjoyable part of this job?</strong></p> <p><em>The most enjoyable part of this job is meeting people from all over the world, to learn from their cultures, caring and assisting for their medical needs, and to provide genuine care and comfort for my patients’ sicknesses while abroad. This type of care, for me, is a totally different approach to nursing compared to that in a hospital setting.</em></p> <p><strong>Where did you gain your experience and knowledge in the field of nursing?</strong></p> <p><em>I was born in Caracas, Venezuela, but came to the United States and became a citizen in 1994. I am extremely proud to be an American citizen. I graduated from Gordon College in Barnesville, Georgia. I have experience in Geriatrics, Psychiatric, Medical Surgical, Counseling Rehab, Spinal injuries, Wound Care, and Trauma. I have worked in a hospital ER setting for the past thirteen (13) years, and as a flight nurse for the last five (5) years.</em></p> <p><img style="display: block; margin-left: auto; margin-right: auto;" src="http://www.commercialmedicalescorts.com/blog/image.axd?picture=2013%2f7%2fIsabel+Pic+!.jpg" alt="" width="400" height="500" /></p> <p><strong>What has been your most interesting transport? Why is that?</strong></p> <p><em>My most interesting transport was from Aruba to Vancouver, Canada. I assisted a client with an open wound to his groin which required a wound vac during the transport. Surgery was impossible at that particular time due to the wound placement. I had to use my wound care expertise and some improvisation, to provide a safe and comfortable, as well as private, environment to care for my patient on such a long trip. This client was a high risk for total amputation (castration) due to the severity of the wound. Luckily, the client arrived at his destination with no further problems thanks to the support and care he received from the CME team. After every transport I follow up with my patient to see how they are doing. This patient in particular had a complete recovery, and amputation was not necessary. I still talk to this client and his family periodically. </em></p> <p><strong>Please describe a challenging trip and the outcome of it?</strong></p> <p><em>My most challenging trip to date was a transport from Italy to New York City. My client became very ill after landing. As I was transporting him in a wheelchair toward the passport checkpoint, his heart rate dropped significantly low, and he became breathless. I activated the call to 911 and asked for an AED, but no one came to assist immediately. I was able to manage getting him to the Immigration point, and successfully restored his breathing. At the same time, I was somehow able to calm his extremely panicked wife. The client survived the vasovagal episode. </em></p> <p><em>When assistance from my 911 call arrived to take him to the nearest ER, the patient totally refused their help. His comment to them was “I don’t need you now! When I was breathless and sick earlier you didn’t come to my rescue! Why do you think you are going to tell me what to do now?” The patient’s vital signs were stable and within normal limits. The patient was then safely transported by the CME team to his local emergency room, where his physician was waiting. I transferred care over to the doctor and ER team in stable condition.</em></p> <p><strong>When you are not flying what do you do?</strong></p> <p><em>When I am not working for CME, I am working full time in an ER at a local hospital. On my days off, I care for my 81-year old mother who lives with me. I also enjoy going to the gym, and spending quality time with my husband.</em></p> <p><em>Before I became a nurse, I was already addicted to flying and traveling. I was a flight attendant for the Venezuelan Air Lines for a period of fourteen (14) years. Since I was four years old, there were two jobs that I dreamed of: being a nurse, and flying!
